Cost Management Strategies and Financial Performance in Pharmaceutical Retail Chains: Evidence from South Indian Markets

Abstract

The pharmaceutical retail sector in India has undergone rapid structural transformation over the past decade, driven by the proliferation of organised retail pharmacy chains, the expansion of generic medicine policy, the entry of e-pharmacy platforms, and intensifying price-based competition. Effective cost management has emerged as a critical determinant of financial sustainability and competitive advantage for retail pharmacy chains operating in this dynamic environment. This study examines the cost management strategies adopted by pharmaceutical retail chains in South India and their impact on financial performance, drawing on secondary financial data from 35 listed and unlisted pharmacy chains and primary survey data from 60 store managers and finance executives. Findings reveal that inventory optimisation and supplier negotiation are the most widely practised cost management strategies, while technology-enabled supply chain integration and centralised procurement yield the highest improvements in gross margin and return on investment. Regression analysis confirms that cost management capability is a significant positive predictor of net profit margin (beta = 0.54, p<0.001). The study proposes a Cost-Performance Optimization Framework tailored to the operational realities of South Indian pharmaceutical retail, providing actionable strategic guidance for pharmacy chain managers, investors, and policymakers.
