In highly regulated life-sciences environments, risk-mitigation strategies have historically prioritised structural, normative, and legalistic paradigms such as Current Good Manufacturing Practices (cGMP), Out-of-Specification (OOS) procedures, and statutory compliance (e.g., Prevention of Sexual Harassment-PoSH). While these frameworks guarantee baseline procedural adherence, they frequently fail to prevent catastrophic failures rooted in organisational silence, latent errors, and defensive psychological routines. This review paper articulates a transdisciplinary socio-technical framework synthesising behavioural safety science, psychosomatic neurobiology, transactional analysis, and non-linear systems dynamics to explain how interpersonal fear impairs industrial reliability.
We examine Heinrich’s safety pyramid within analytical chemistry and drug manufacturing workflows, demonstrating how the suppression of low-level operational anomalies directly escalates the probability of critical regulatory citations and compromised patient safety. Integrating physiological stress models with the psychodynamics of organisational inertia, we illustrate how threat perception induces cognitive tunnelling, defensive rationalisation, and systemic friction. Furthermore, we outline the behavioural physics of cultural transition (ΔE) and map organisational habit formation against neuro-physiological renewal intervals (21, 40, and 108 days). Finally, we provide an operationalised taxonomy of leading versus lagging cultural metrics, repositioning psychological safety from an auxiliary human resources initiative to an indispensable prerequisite for pharmaceutical quality, compliance integrity, and patient protection.
